Methodology
Tests for high-risk HPV types 16, 18, 31, 33, 35, 39, 45, 51, 52, 56, 58, 58, 66, and 68. Testing performed on the Roche cobas 4800 System that utilized Polymerase Chain Reaction (PCR) and nucleic acid hybridization.

Specimen Type: Vaginal or cervical smear
Container/Tube: ThinPrep Collection Kit
Collection Instructions:
1. Collect vaginal or cervical smear using a ThinPrep® Collection Kit. See link for collection instructions.

2. Label container with patient’s name, date of collection, and type of specimen.
3. Please complete a “Histopathology/Cytology” request form in Special Instructions and forward it with the specimen.
4. All requisitions must include the following information: patient’s first and last name; doctor’s name; patient’s age; patient’s address; and date of collection.
5. Indicate source of specimen.
