Specimen Requirements

Specimen Type: Vaginal or cervical smear

Container/Tube: ThinPrep Collection Kit

Collection Instructions:

1. Collect vaginal or cervical smear using a ThinPrep® Collection Kit. See link for collection instructions.

2. Label container with patient’s name, date of birth, date of collection, and type of specimen.

3. Please complete a “Histopathology/Cytology” request form in Special Instructions and forward it with the specimen.

4. All requisitions for Pap smears must include the following information: patient’s first and last name; doctor’s name; patient’s age; patient’s address; date of patient’s last menstrual period, pertinent obstetric and gynecologic information (pregnant, postpartum, hormonal therapy, radiation therapy [type and date], surgical therapy [type and date]), history of past abnormal Pap smear; and date of collection.

5. Indicate source of specimen.
