Specimen Requirements

Container/Tube: Pink-top (EDTA) tube

Specimen Volume: Full tube of blood

Reject due to hemolysis: YES

Collection Instructions:

1. Patient must be identified by verifying patient name, date of birth, and 6-digit medical record number from Holland Hospital (HH) identification band on patient’s arm. Note for Inpatients: Do not draw patient if there is no HH identification bracelet.
2. If patient is alert and oriented, have patient confirm spelling of first and last names. If unable, a family member may verify spelling.

3. Draw a minimum of 3 mLs of blood for a pink EDTA tube, according to appropriate venipuncture procedure.

4. Obtain the two barcoded patient labels from PDA. Place a label onto the "Place patient information here" area of the Typenex band; cover the label with clear shield on the band. 

5. On the same arm as the HH ID bracelet, wrap TypenexTM band once around patient’s wrist. Snap closure on bracelet to secure and remove bracelet sticker tails at perforation.
6. Label specimen with both PDA barcoded patient label, and with TypenexTM barcode sticker.

7. Collect tube with PDA, scanning both barcodes into LIS.
