Container/Tube: Serum gel tube

Specimen Volume: 1 mL of serum

Collection Instructions: "Vancomycin levels are ordered 15 minutes prior to the infusion (trough), with morning labs or after dialysis (hemodialysis patients) or 2 hours after the completion of infusion depending on the population."

Note: 1. Indicate serum and peak or trough.

2. Label specimen appropriately (serum and as peak or trough).
